Direct State Aid Online Report Form FY12

The preferred method is to submit your report electronically - due July 31, 2012.

 

Report on Direct State Aid funding received and spent between July 1, 2011 and June 30, 2012.  Under the broad categories, describe how the funding was used and list the amount spent in that category. Boxes marked with a red dot are required.

When finished, click the submit button at the end of the report to submit your data to Iowa Library Services. You will receive an email verification.
