Which forms do I need to send in to participate in Access Plus and receive Access Plus reimbursement in FY09?

To participate in Access Plus in FY09, the following forms must be filed:

• The Access Plus Annual Report (due August 31, 2008)

• The Access Plus Transaction Log, if applicable (due August 31, 2008)

• The Enrich Iowa Letter of Agreement for FY09 (with Access Plus checked)
