Computer Class Lesson Plan form

This allows libraries to share their computer class lesson plans through the Information Literacy Toolkit.

This information will be posted to the Computer Lesson Plans area of the State Library's Information Literacy Toolkit. Fill out all the information as completely as possible and attach all pertinent documents (PowerPoint, Word, Excel). Please only submit lesson plans that have been taught at least once.

As it was marketed to the public.

Class description (Required)

Please use the description that was used to market it to the public.

Learning Objectives of the class (Required)

Please number each objective.

Class files: PowerPoint

Please upload the PowerPoint if one was used with the class.

Class files: agenda or outline (Required)

Please upload the class agenda or outline (Word or Excel).

Class files: handouts (Required)

Please upload any handouts used with the class (Word preferred). These will need to be uploaded as one file.

Class files: instructor notes

Please upload any instructor notes and tips (Word preferred).

I give the State Library permission to publish submitted information, if they so choose. (Required)

Please check this box if you will allow the State Library to post your information on your class. The State Library reserves the right, however, to not post any or all of the submitted material.
