Use this form to submit your emergency contact information so we can contact you in the event that a PLOW training session is canceled.

Please enter a phone number where State Library staff can reach a contact person for your PLOW Project participating library in case a  PLOW project training session is canceled or changed.

The State Library is collecting this information so we can quickly notify  participants in the event of bad weather or instructor sickness or any such thing. We don't think this will happen, but we want to be prepared.
