Survey process for FY 2004

At the survey web page, click on "Enter the Iowa Library Media Website." A login box will appear. Enter the survey password (no user name required) and click on "OK." The password is slmc3 (lower case).

A web page entitled "Find Your Survey" will appear, with a password box. Enter the School password (LS . . .) and click on "Continue." Click on the hyperlinked school name that appears on the web page. Please check the pre-filled information from FY 2003 and update for FY 2004 (July 1, 2003-June 30, 2004) as needed.

Complete part or all of the survey. When you are ready to submit, go to the end of the survey and click on "Submit Survey." A web page with the text "Survey Saved" will appear. To perform calculations, use the web browser's back button to return to the survey, then use the recalculate function of the browser (F5 for Internet Explorer, Ctrl and R for Netscape). You may return to the survey several times if needed, just remember to click on "Submit Survey" at the end of each session.

When you have completed the survey, check the "Survey Complete" box in Part L.

The survey may be printed using the Print function of your web browser.
