I agree to submit the following to the Continuing Education consultant at the end of the project: 1. A self-evaluation of the original plan, the process and the outcomes of objectives. 2. A representation of the project for review ( a link if it is online publically, screen shots with explanations if online privately, copy of anything written, etc.)
